Clinic Policies

Insurance Policies
If you have insurance that covers acupuncture we will gladly submit your claims for you. You are responsible for your deductible, your co-pay and co-insurance amounts. Please verify that your insurance is current, that it will cover the medical services provided by a Licensed Acupuncturist, and that you have met your deductible. If your insurance denies payment of a claim, or does not cover a specific service provided by Heart of the Forest Acupuncture, you are responsible for the billed charges.

– I am a preferred provider for Premera, Lifewise, and Aetna Insurance companies. This means Heart of the Forest Acupuncture will submit your claims.**
-If you have insurance that I am not a preferred provider, we will not be able to bill your insurance company for you but we would be happy to provide you with a “super bill” that you can send in to your insurance company for reimbursement. Many insurance plans will cover a percentage of “out of network” claims – check with your insurance company to see how much they will reimburse.
-If you arrive for your first appointment and it turns out either acupuncture is not covered or I am not a preferred provider, and you choose to cancel the appointment, the cancellation policy will be in effect and you will be charged the $50 cancellation fee.

-Please note that we will bill some insurance companies as a courtesy to you but that the ultimate financial obligation is yours and not that of your insurance company. Our insurance rates are typically $132 for an Initial Visit and $90 for a Return Visit, though it may vary depending upon specific treatment (please see fee schedule details below). These rates differ from our time of service payments which are discounted. Please note that these are the fees that you will be charged if you need to meet your deductible before your coverage begins. If you have further questions about this process please do not hesitate to ask us, we would be happy to clarify.

**Insurance will not cover the cost of herbs.

**Apple Health/Molina and Medicare currently do not cover acupuncture.

**Insurance will not cover missed appointments fees and you will be charged $50 for rescheduling or cancelling your appointment that was reserved for you without 24 hours notice.

If you have a Health Savings Account (HSA): Acupuncture treatment is allowable under all HSA’s. You will need to check with your specific HSA to find out if herbal medicine prescribed by a health professional is an allowable expense.

*If needles are an issue for you or your child, Chinese medicine can still help! I offer alternative treatments tailored to you and/or your child’s needs and may include one or more of the following: Japanese style non-insertive needling, moxa, cupping, guasha, acupressure/shiatsu massage, aromatherapy, ear seeds, herbal and dietary therapy, and lifestyle counseling. Mentorship and/or shamanic medicine are also powerful modalities that can help you on your path to good health. Please note this type of treatment is not billable to insurance.

-I make available 2 sessions per month at a sliding scale rate which begins at $25. Please call if you would like to discuss this option and/or to be placed on the waiting list.

-I offer payment plan options so that you can receive the treatment you need with the flexibility to pay over a period of time. Please call and we can discuss your individual situation.

Cancellation Policy
We ask that you provide Heart of the Forest Acupuncture with at least 24 hours notice if you need to reschedule or cancel an appointment. There is a $50 fee for missed appointments that will be added to your account. If you are too sick*, have sick family members, or if there is an emergency or inclement weather, don’t worry, there is no charge.

*If you are sick, but are able to to come to the clinic, acupuncture and herbs can help you feel better! Just call/email and we can determine if this is a good choice for you.

 

Office Policies

We are available for communication primarily via phone and email. We do our best to return phone calls and emails within 24 hours. I am available for communication from Monday to Thursday with active patients for quick questions that can be answered briefly by phone or by email. Medical issues are best managed by making an appointment and having an in-person consultation.

I do not practice emergency medicine. If you have an emergency, please call 911 or report to your local emergency room. If you have an urgent situation that you think does not require an emergency room visit, and you think acupuncture may be able to help, please call. We will do what we can for you, but if your situation worsens or if you do not hear back from us within a time-frame that is appropriate for your situation, please do not hesitate to call 911 or report to your local emergency room.

Patients who show inappropriate conduct, non-or late payment of fees, or safety concerns may be denied service.
